Data fields include Number, Period, Name, Regular Hours, Rate, Amount, Overtime Hours, Rate, Amount, Deductions, Actual Pay, Pay Date, Employee Signature, Supervisor Approval, and four columns for two IN/OUT Punch cycles with Total Column. The model comes as 1000 cards per pack.
